Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our commitment
JTS Medical, PLLC ("we," "our," "the practice") is required by law to maintain the privacy of your protected health information ("PHI"), provide you with this Notice of our legal duties and privacy practices with respect to PHI, and follow the terms of the Notice currently in effect.
How we may use and disclose your PHI
- Treatment: to provide, coordinate, or manage your care and any related services, including consultations and referrals.
- Payment: to bill and collect payment from you, an insurer, PIP or workers' compensation carrier, or a responsible party, including generating itemized invoices and superbills.
- Health care operations: for quality assessment, credentialing, training, licensing, and general practice administration.
- Appointment reminders and follow-up: we may contact you by phone, text, or email using the contact information you provide.
- As required by law: including public health activities, reporting abuse or neglect, health oversight, judicial proceedings, law enforcement, and to avert serious threats to health or safety.
- Workers' compensation and auto (PIP) claims: to the extent authorized by and to comply with applicable law.
Uses and disclosures that require your authorization
Most uses and disclosures of psychotherapy notes (where applicable), uses for marketing, and disclosures that constitute a sale of PHI require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.
Your rights
- Inspect and obtain a copy of your PHI.
- Request an amendment to your PHI.
- Request an accounting of certain disclosures.
- Request restrictions on certain uses and disclosures. We will accommodate a request to restrict disclosure to a health plan when the service has been paid in full out-of-pocket.
- Request confidential communications at an alternate address or number.
- Receive a paper copy of this Notice, even if you agreed to receive it electronically.
- Be notified in the event of a breach of unsecured PHI.
Our duties
We are required by law to maintain the privacy of PHI, provide you with notice of our legal duties and privacy practices, and abide by the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the new terms effective for all PHI we maintain. Revised notices will be posted on this page.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with the practice or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Acknowledgment
At your first visit, we will ask you to acknowledge receipt of this Notice. If we are unable to obtain a written acknowledgment, we will document our good-faith effort.
