Financial Policy
JTS Medical is a private, fee-for-service practice. This policy explains how visits are billed and your rights under the federal No Surprises Act.
Fee-for-service practice
JTS Medical is not in-network with commercial insurance plans or Medicare. Payment is due at the time of service. We accept cash, check, credit and debit cards, HSA, and FSA.
Upon request we can provide an itemized receipt or superbill that you may submit to your insurer for possible out-of-network reimbursement. Reimbursement is not guaranteed and is between you and your insurer.
PIP and workers' compensation
For qualifying auto injuries covered by Personal Injury Protection (PIP) and for workplace injuries covered by workers' compensation, we can bill the responsible carrier directly. Patients are responsible for any balance not paid by the carrier.
Missed appointments and cancellations
Please notify the office at least 24 hours in advance if you must cancel or reschedule. Missed appointments or late cancellations may be subject to a fee, which will be disclosed to you in advance.
Your right to a Good Faith Estimate (No Surprises Act)
Under Section 2799B-6 of the Public Health Service Act, health care providers and facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or who are not seeking to file a claim with their plan or coverage, both orally and in writing, of their ability, upon request or at the time of scheduling health care items and services, to receive a "Good Faith Estimate" of expected charges.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill through the federal patient-provider dispute resolution process.
- Make sure to save a copy or picture of your Good Faith Estimate.

Balances and collections
Unpaid balances more than 60 days past due may be referred to a collection agency. You are responsible for reasonable collection costs, including attorneys' fees, permitted by law.
